THE CORRELATIVE RELATION BETWEEN THE NEUROPEPTERGICAL AND SIMPATICOADRENAL SYSTEMS IN PERIOPERATIVE PERIOD DURING SOME ACUTE ABDOMINAL PATHOLOGIES

The aim of this study was to learn the correlative relations between neuropeptergical and simpaticoadrenal systems in the patient with acute bowel obstruction, cholecistopancreatitis, stomach and duodenal ulcer, complicated with gastro-intestinal bleeding. The study was carried out in 90 patients which were divided into 2 gropes. The patients from first grope, which contained 60 person, were given narcotic analgesics, when the patients from second grope (30 patients), took narcotic analgesics and dalargiin. In addition to the routine investigations, the adrenalin and endorphin blood concentration were realized as well. The investigations were carried out in 3 stages. First stage before operation, second stage in the first day after operation and finally third stage, in the third day after operation. The analyses of the obtained results had been shown, that there is a direct correlative link between the neuropeptergical and simpaticoadrenal systems. The duration of the analgesia in the patients from the first grope was 4 – 6 hour, but in the patients from the second grope was 8 – 12 hour. The adequate analgesia in the perioperative period is necessary to prevent exhausting of opiod system, and to keep homeostasis. Investigation peculiarity of correlation between helps to choose more optimal version of general anesthesia.
INFLUENCE OF ANTICOAGULANT THERAPY ON THE CYTOKINE STATUS IN THE COMPLEX TREATMENT OF SURGICAL DISEASES

Anticoagulant therapy regulates not only the hemocoagulation system, but also the immune mediators – cytokines, contained in blood serum and creating conditions for the formation of thromboses. Taking this into consideration, we set a purpose to study the influence of direct anticoagulant – sulodexidine (Angioflux) on the level of cytokines. The investigations were carried out in 40 patients exposed to surgical operation because of various surgical pathologies of abdominal cavity. The patients were at the age of 40-80. The patients were studied in 2 groups: 20 patients that entered to the 1st group (comparative group) did not undergo “Angioflux” prophylactics in addition to non-specific measures. 20 patients that entered to the 2nd group (main group) underwent sulodexide content “Angioflux” prophylactics in addition to non-specific prophylactic measures. 10 actually healthy persons were taken as a control group. The results of conducted examinations showed that before the operation the ŞNFα content in the comparative group was 11,2 times (p<0,001), and in the main group 10,7 (p<0,001) times more as compared to the indicators of actually healthy persons. On the 1st postoperative day in the comparative group the content was 5,0 (p0<0,001) times, and in the main group 2,5 times (p0<0,001) higher than in the preoperative period. On the 7th postoperative day the ŞNFα content in the comparative group was1,7 times (p0<0,01) higher than in the preoperative term, and in the main group was close to the preoperative indicators. Before the operation the IL1β content in the comparative group was 9,7 times (p<0,001), and in the main group 9,6 times (p<0,001) times more as compared to the indicators of actually healthy persons. On the 1st postoperative day in the comparative group the content was 1,4 times, and in the main group 1,2 times higher than in the preoperative period. On the 7th postoperative day the IL1β content in the both groups was lower than in the preoperative term. However, as compared to the actually healthy persons, the indicators of blood serum in comparative group were 8,3 times (p<0,001), and in the main group 6,1 times (p<0,001) higher.
Thus, the results of conducted investigations showed that by the on the 3rd and 7th postoperative days the application of “Angioflux” in the complex treatment of patients truly reduces the content of cytokines that increased on the 1st postoperative day. The fact shows, that in addition to anticoagulant effect, the suledxide (Angioflux) has anti-inflammatory action.
