THE POSTOPERATIVE ROLE OF CRP AS A MARKER OF INFECTION

IN PATIENTS UNDERGOING CABG SURGERY

Introduction: The normal amount of CRP as inflammation marker in blood is 0-6 mkg. CRP may increase up to 500 mkg not related to the location of inflammation .

Aim: The aim of this study is to evaluate postoperatively the level of CRP in patietns who underwent on-pump CABG in normal and infectiously complicated cases.

Results: In 100 patients who underwent on-pump CABG were determined CRP levels in blood after 1 or 2 days. The patients with high CRP due to another reasons excluded from the study. All patients divided into 2 groups according to the level of CRP. 86 patients were included in group I . The level of CRP in this group in second day was 6.5-48.9, third day was 4.6-31.2 and seventh day was 5.1-14.5. Non-infectious complications appeared in this group. Results of blood samples were negative. Patients were discharged in 6-9 days. In patients included to the group II (14 patients) were seen high CRP levels on the second postoperative day. Also other indicators of inflammation were elevated in this group (the amount of leukocytes, procalcitonin, high fever). In all patients in this group antibiotic treatment tactics were changed . The effectiveness of antibiotic treatment was controlled by daily determination of CRP levels. If there was no increase in CRP level antibiotic therapy is untouched, otherwise the antibiotics is changed. One death occurred in this group.

Conclusions: We believe that the CRP has an important role as inflammation marker in control of effectiveness of antibiotic treatment.
