METHOD OF SURGICAL TREATMENT OF COLOPROCTOGENIC COLOSTASIS IN PATIENTS WITH DOLICHOCOLON

Method of single-stage combined surgical treatment of chronic coloproctogenic constipations by means of Kummel-Zerenin rectopexy in combination with total colectomy was offered (patent No. 2414861 for invention «Method of surgical treatment of chronic coloproctogenic constipation», priority date 11.03.2010, date of publication 27.03.2011). Chronic colostasis on the background of dolichocolon with slowing down of the transit of large intestine contents for more than 96 hours, megarectum and indication in the anamnesis on long-term and ineffective conservative therapy or earlier unproductive resection of large intestine in combination with internal prolapse of rectum. 3 patients with good direct results and follow-up were operated by the offered method. To our mind, efficiency of Kummel-Zerenin rectopexy in combination with total colectomy is provided: 1) by removing nonperistaling elongated and atonic large intestine by its total resection, by the provision of free passage of semifluid contents of ileum through ileorectoanastomosis into rectum, that excludes the probability of constipations, but provides regular soft or half- liquid stool; 2) by removing pathologic submucosal-mucosal fold in the lumen of the rectum (internal prolapse or internal invagination) through its movement from the cavity of small pelvis in the proximal direction for the prevention of recurrence of anorectal obstruction; 3) by removing deep rectouterine (rectovesical) bag of the peritoneum for the prevention of recurrence of the prolapse and falling of rectum and retention of passage of its contents.
