FEATURES ON TREATMENT OF SEPTIC WOUNDS AT MIDDLE AGED AND OLD AGED PATIENTS

Introduction Wound healing is more long-termed at middle aged patients than at younger people. One of its explanations is slowdown of physiological and biochemical processes in the organism as the result of reduced activity of gonadal hormones, especially estrogen and defective functioning of cellular membrane. Thus, utilization of estrogens and metabolic preparations is the perspective direction to complex treatment of wounds at the patients of the same category.

Materials and methods 116 middle aged and old aged patients having pyoinflammatory diseases of soft tissue have been cured by use. All patients have been operated and undergone to a standard post-surgical treatment. The followings were included additionally in the schema of the standard treatment for patients of the principal group (55 patients): “mexidol” intramuscular drop-by-drop introduction, “lipin” intramuscular drop-by-drop introduction, estradiol transdermal in the therapeutic doses in the first phase of injured process, application of wounds by mexidol-liposome mixture. The wound healing process was controlled clinically, cytologically and planimetrically. The statistic processing of obtained results was implemented using the program SPSS for Windows 17,0 (SPSS inc.).

Results A statistically meaningful decrease of duration of pain syndrome to 1,8±0,84 a day, edema of paravulnar tissues to 2,4±0,67 a day, acceleration of wound cleansing to 1,9±0,78 a day, occurrence of active granulations to 1,7±0,89 a day, healing of wounds to 2,2±0,68 a day, 1,6±0,67 times increase of planimetric index was marked at the patients of the principal group in comparison with the control group. Decrease of number of neutrophilic granulocytes to 12,1% and their destructive forms to 35,7%, increase of number of fibroblasts to 64,8%, phagocytic activity to 10,8%, decrease of number of destructive and increase of regenerative cytogram.

Conclusion Utilization of metabolic preparations and estrogen in complex treatment of wounds at the middle aged and old aged patients leads to activation process of phagocytosis, normalization of inflammatory reaction of tissues, reinforcement of repair processes and acceleration of healing of wounds.
