Thank you for demonstrating your interest towards the IV Advanced Leadership School (ALS). These guidelines are designed to assist you with applying for participation in this project.

The Application Form consists of two parts:

• Primary Applicant Information,
• Reference Form.

Only those applications that include all the two components will be considered.

While completing Primary Applicant Information, please answer all the questions in a sincere way. If a question is not applicable, indicate N/A (non-applicable). This component includes several questions to consider as well. It is very important for us to be aware about your attitude towards IV ALS and MUNRFE. Do not leave any space blank. Provide all the details concerning your contact information: it is very important for us as we will need to contact you once your status is determined.

A Reference Form should be completed by an evaluator you refer to. More detailed instructions are contained in the reference form. You do not need to stamp the reference form. Please make sure that the contacts of evaluator are correct: we will need to contact him/her while considering your application. Attention: in case of any sort of cheating the applicant will be automatically banned from his participation in the IV ALS. Such decisions of the IV ALS Coordinators are not subject to any appeal.

A full application form should be e-mailed to savenkov_doc@mail.ru by January, 12, 2012, 23:59 (UTC +10:00). Please name your application file by adding your last name to the current name of the application form (for example:IV_ALS_Application_Form_Savenkov).
