INFLUENCE OF ARTIFICIAL VENTILATION OF LUNGS (AVL) ON THE INDICATORS OF IMMUNITY IN POST-OPERATIVE PERIOD DURING GUN-SHOT PERITONITISES

Despite recent significant results obtained in the treatment of acute spread peritonitises including gun-shot peritonitises, the mortality rate still remains high and according to different authors ranges between 15 – 85%, because incomplete correction of the changes occurring in the organism both before surgery and after surgery: intoxication, hypovolemia, hemodynamic and metabolic changes, respiratory compromise etc. Use of artificial ventilation of lungs at difference stages of treatment of peritonitis plays one of the main roles in the improvement of results. Therefore, the main purpose of the investigation was to study the influence of influence of artificial ventilation of lungs on the indicators of immunity in the complex treatment of peritonitis formed during gunshot wounds.

To achieve the abovementioned purpose, 45 patients with gunshot peritonitis divided into 2 groups were examined. The first group included 15 patients (control group). Artificial ventilation of lungs was not used in the post-operative period of treatment of these patients. 30 patients constituting the second group were not separated from artificial ventilation after the completion of operation and they underwent artificial ventilation of lungs in different respiration modes within 6 hours. Indicators of gas content and acid-base balance in arterial, venous and capillary blood of all the patients were studied by the Astrup’s micromethod. Moreover, indicators of immunity in all the patients of the two groups were analyzed.

In the result of conducted studies it was determined that based on the indicators of immunity of the second group, studied in the closest period after the operation the amount of “nonsludging”, “early”, “total” rosettes increased, in the amount of stable rosettes there were no changes, and the amount of “restored”, “autolysed” rosettes increased as compared to the first group. The relation of T- lymphocyte inductor-helpers to suppressor-killers was high in the second group.

The investigations allows to conclude that purposefull issue of ASV is of treatment and prognostic significance till the stabilization of hemodynamics and gases in gun-shot peritonitises. If to consider that the state of patients of the main group was poorer than the state of the control group, then the significance of post-operative measures is higher.
