INTRAVENOUS LASER (BΛOK-405) IRRADIATION OF BLOOD IN COMPLEX TREATMENT OF DIFFUSE SUPPURATIVE PERITONITIS

Purpose of the research: Improvement of the results of complex treatment by using the method of intravenous laser (BΛOK-405 n.m) blood irradiation in the treatment of endogenous intoxication syndrome observed in patients with diffuse suppurative peritonitis.

Materials and methods: 160 patients were included to the research. The patients applied to the hospital urgently. 87 patients were entered to the hospital within a day of the start of the disease, 34 patients on the second day, 26 patients till the third day and 13 patients being later than three days. 54 patients were included to the control group. These patients were treated by the traditional method during the post-operative period. (Antibacterial, dezintoxication, decompression of gastrointestinal tract and so on). Method of intravenous laser (Matrix-BΛOK) blood irradiation was applied to 106 patients included into II group over 5-7 days depending on the degree of severity of the illness during the post-operative period along with complex treatment measures. Bacteria and the toxins secreted from bacteria create endotoxicosis by being sucked in blood and lymph vessels and it is one of the main reasons determining development and course of peritonitis, aggravating the disease.The degree of severity of the endogenous intoxication depends on the character of destruction in inflammation furnace, development degree of enteral deficiency, pathogenic degree of microorganisms.Irrespective of application of high-level surgical technique, complex treatment measures, Endogenous Intoxication Syndrome arising during suppurative peritonitis remains as the main cause leading to death..The patients were divided into 3 groups in accordance with the degree of severity of the endotoxicosis. 46 patients with light endotoxicosis symptoms (I degree), 51 patients with medium endotoxicosis symptoms (II degree) and 63 patients with severe endotoxicosis symptoms (III degree) were under our observation. The degree of severity of the intoxication was estimated according to the V.Ê.Gostishev and B.Ê.Shurkalin’s classification. Resting on general criteria of endotoxicosis and the results of the laboratory analyses, we came to the conclusion of exasperations that may happen during the post-operative period in patients treated with intravenous laser irradiation and decline in death rates. Enteral deficiency syndrome was observed in 14 (26.6%) patients, bowel interloop abscesses in 2 (3.7%) patients, lethality indicator in 8 (14.1%) patients among the patients included to the control group. Enteral deficiency syndrome was observed in 29 (28.8%) patients, bowel interloop abscesses in 6 (7.4%) patients, lethality indicator in 10 (16.1%) patients among 106 patients included to the main group.

Conclusion. The researches confirmed that together with complex treatment of the patients with diffuse suppurative peritonitis (detoxication, antibacterial, methods of combating with enteral deficiency, adequate sanitization, etc.) intravenous laser irradiation of blood (405 n.m) plays an important role during 5-7 days, depending on the degree of severity of endotoxicosis, in reducing aggravations and lethality indicators observed in post-operative period.
